අයදුම්කරුගේ ප්‍රකාශය :- මා විසින් මෙම අයදුම්පතෙහි සඳහන් කර ඇති සියළුම විස්තර සත්‍ය බවත්, නිවැරදි බවත් මෙයින් සහතික කරමි. මා අ.පො.ස. (සා.පෙළ) සමත් බවත් භාෂාව, විද්‍යාව, ගණිතය යන විෂයන්ට හා තවත් එක් විෂයකට සම්මාන සාමාර්ථයක් ඇති බවත් හා උසස් පෙළ ජීව විද්‍යා විෂයධාරාව/ තාක්ෂණවේදය විෂයධාරාව හදාරා ගැසට් නිවේදනයේ නම් කර ඇති සියළුම සුදුසුකම් සපුරා ඇති බවටත් සහතික වෙමි. මෙහි සඳහන් කරුණු අසත්‍ය බව හෝ වැරදි බව හෝ හෙළි වුවහොත්, මා තරඟ විභාගවලට පෙනී සිටිමට හා කෘෂිකර්ම විද්‍යාලයට තෝරා ගැනීමට නුසුදුස්සකු වන බව මම දනිමි. විභාගය පැවැත්වීම හා ප්‍රතිඵල නිකුත් කිරීම සම්බන්ධයෙන් කෘෂිකර්ම අධ්‍යක්ෂ ජනරාල් විසින් පනවනු ලබන නීතිරීති වලට මම යටත් වන බවද ප්‍රකාශ කරමි.

விண்ணப்பதாரியின் உறுதிமொழி இவ் விண்ணப்பப் படிவத்தில் என்னால் வழங்கப்பட்டுள்ள அனைத்து தகவல்களும் உண்மையானவையும், சரியானவையும், என இத்தால் உறுதிப்படுத்துகிறேன். நான் கா.பொ.த சாதாரண தரம் சித்தியடைந்துள்ளேன் எனவும், மொழி, விஞ்ஞானம், கணிதம் ஆகிய பாடங்களுக்கும், மேலதிகமாக இன்னும் ஒரு பாடத்திற்கும் திறமை சித்தி அடைந்துள்ளேன் எனவும் , கா.பொ.த உயர் தரத்தில் உயிரியல் பாடத்தில் / தொழில்நுட்ப பாடத்தில் கல்வி பயின்று வர்த்தமானியில் குறிப்பிடப்பட்டுள்ள அனைத்து தகைமைகளையும் பூர்த்தி செய்துள்ளேன் எனவும் உறுதியளிக்கிறேன். இங்கு குறிப்பிடப்பட்டுள்ள தகவல்கள் பொய்யானவை என நிரூபிக்கப்படும் எனில் நான் போட்டிப் பரீட்சைக்கு தோற்றுவதற்கும் , விவசாயப் பாடசாலைக்கு தேர்ந்தெடுப்பதற்கும் தகுதியற்றவர் என நான் நன்கு அறிவேன். பரீட்சை நடத்துதல் மற்றும் பெறுபேறுகளை வௌியிடல் தொடர்பாக விவசாயப் பணிப்பாளர் நாயகத்தால் மேற்கொள்ளப்படும் விதிமுறைகளுக்கு நான் கட்டுப்படுவேன் என உறுதியளிக்கிறேன்.

Declaration of Candidate:-I do hereby declare that all the particulars furnished by me in this application are true and correct. Further, I affirm that I have passed the G.C.E. (O/L) Examination with credit passes for the subjects of Language, Science, Mathematics and one other subject and satisfied the requisite qualification mentioned in the Gazette Notification, having studied the stream of Biology/ Technology for the G.C.E. (A/L) Examination. I am aware that I am liable to be disqualified for the Competitive Examination and for admission to the Schools of Agriculture if it is revealed that the particulars given in the application are false or incorrect. Further, I hereby agree to abide by all the terms and conditions imposed by the Director General of Agriculture in respect of conducting this examination and releasing results of the examination.
